Let's make it Personal - Cheshire County Council are actively seeking people's views on some important changes to the way that Adult Social Care will be delivered in the future.

Personalisation of Services is a new way of providing social care services. It recognises that people are the best judges of their own needs and focuses on what is important to individuals and the outcomes they want to achieve. It will offer people greater choice over the support they receive and will help them develop better links with their local communities, enabling them to live as independently as possible.

Under Personalisation of Services, instead of a package of care being compiled by Community Services following an assessment, those adults whose needs are assessed as critical or substantial will be offered a Personal Budget with which they can design their own package of care. People will be asked to complete a Joint Assessment form which will pose a range of questions relating to their personal and social needs. The answers to these questions are then converted into numerical points indicating the level of support a person needs. These points are then converted into an allocated sum of money, the Personal Budget. After a financial assessment has been undertaken, to see whether people will need to make a financial contribution, people will be informed of the value of their Personal Budget. People can then decide, with help if they need it, on the type of support and care they would like to spend their Personal Budget on. Individuals will be asked to write up a plan of their support and agree with the Council how their care needs are to be met.

Cheshire County Council are also going to invest more money into preventive and re-ablement services which aim to improve people's health and wellbeing so that they require less social care services in the future.

This new model is likely to involve changes to the way the Council will charge for services and the way in which the costs of services will be calculated. The Council, therefore, need your comments to ensure the views of existing and new users of the service are taken into account before final decisions can be made.
