Please complete and submit the form below to register for your child to receive a Cheshire Primary School Application Form.  

You will then receive an application pack in September 2008 containing the application form and information on how to apply for school places in Cheshire.

You will need to complete and return the application form from your pack if your child was born between 1 September 2004 and 31 August 2005, as your child will be due to start school in September 2009 (or January 2010 if preferred).

If your child was born before 1 September 2004, do not complete this form. Please contact the Admissions Team on 0845 111 0304 for further advice.
