The Highflyers Positive Practice award was in the Information Sharing – joining Health and Social Care category. The awards ceremony was set up by the Care Services Improvement Partnership (CSIP). “I would like to congratulate everyone involved” said Executive Member for Adult Social Services Lynn Hardwick.
“They did extremely well against some strong competition.
“Their work has been so successful that we are hoping to roll it out later this year to people aged eighteen and over.” The group is made up of Cheshire County Council, Central and East Cheshire Primary Care Trust, Western Cheshire Primary Care Trust, Central Cheshire Older Peoples Resource Group and Cheshire Older Peoples Network.
