How do I get help with long term care?

Our Social Care and Health teams provide a range of services for all adults to enable them to live independently and to care for those with special needs whether it be due to a physical or learning disability or due to mental health problems. Contacting our Social Care and Health team for a health and social care assessment is the first step for anyone who requires long term care.

I want to stay in my own home but need help with some tasks

The Council work together with vulnerable people to meet their housing support needs, enabling them to achieve greater independence. Our Community Services department and Housing Authorities can assess what improvements and adaptations you may need in order to live more independently in your own home. Is there any help for my carer?

A carer is someone who looks after a relative, friend or neighbour who is ill or has a disability and would not be able to live in the community without their help. The Council's 'Ask Pat' campaign has been set up to let people who spend a significant amount of their time caring for someone else know that they can get help.

Carers are often unaware of the support available to them. If you think you are one of these people phone 0845 11 333 11 and ask Pat for more information.

How do I find out about Care Homes?

Our Care Homes Directory lists details of all Care Homes within Cheshire and is intended to assist the people of Cheshire, their families or Carers who are making the decision of moving into a Care Home.

Benefits

If you are unable to work or your earnings are low because of caring responsibilities you may be able to get Home Responsibilities Protection (HRP), which will help protect your basic Retirement Pension. Visit the Pension Service website for further information on HRP and Directgov for further information on benefits for those with long term illness or disabilities.

  • Age Concern Cheshire provides a diverse range of care and support services to promote the well being of older people in Cheshire.
  • Altogether Now has news and information service for anyone whose lives are affected by disability and, or age.
  • Citizens Advice Bureau has benefits advice for people who are sick or disabled.
  • Directgov - Caring for someone has lots of information on caring, support, disabilities, money, employment and health.
  • Disability Rights Commission's goal is to create a society where all disabled people can participate fully as equal citizens
  • MIND produce a booklet for older people which explores how older people can maintain good mental health.
  • My Community contains information on doctors, dentists and hospitals in your area of Cheshire.
  • Older Peoples Network is a network for older people, their carers, and any individual or group interested in working to improve the quality of life for older people and their carers in Cheshire.
  • The Samaritans is a national organisation if you need someone to talk to.
  • Welfare Rights Service is a specialist advice service offering confidential, impartial and independent advice on welfare benefits, free of charge.
