Partners

The organisations involved in the Cheshire Health and Social Care Community for the delivery of the Cheshire Single Assessment Process (CSAP) solution are:

• Western Cheshire Primary Care Trust
• Central and Eastern Primary Care Trust 
• Cheshire and Wirral Partnership NHS Trust 

And the three Acute Hospital Trusts based in Cheshire:  

• Countess of Cheshire NHS Foundation Trust
• East Cheshire NHS Trust
• Mid Cheshire Hospitals NHS Trust

There is a CSAP project board consisting of senior management representatives from each partner organisation who take overall responsibility for the project.

The Directors of Finance have also been consulted at each stage of the tendering process.
